Enhanced Care Management
Enhanced Care Management (ECM) is a free Medi-Cal benefit that provides extra support for members with complex health and social needs. Through Molina, you will have a dedicated Lead Care Manager and care team who work closely with you to coordinate your medical and behavioral health care, help manage medications and doctor’s appointments, support you during and after hospital stays, and connect you with important resources such as housing, food, and transportation.
ECM is designed for Medi-Cal members who may need more help navigating care. This includes people experiencing homelessness or housing instability, those with frequent hospital or emergency room visits, individuals living with serious mental health or substance use challenges, adults at risk of entering or transitioning out of nursing facilities, people returning to the community after incarceration, children and youth with complex medical or behavioral needs (including those in foster care), and pregnant or postpartum individuals who need additional support.

Your dedicated Lead Care Manager and support team work closely with you to coordinate all aspects of your care, ensuring you receive the right services at the right time.

Medication Management
We help you stay on track with your prescriptions by coordinating refills, reviewing medications, and supporting safe and effective use.

From scheduling to reminders and follow-ups, our team makes sure you get to your doctor’s visits and receive the care you need.

Social Services Support
We connect you to essential community resources like housing, food, and transportation so your daily needs are supported along with your health.
